Use this page to download and share Rx Pathway (RxP)resources, including individual RxP cards. 

HOW YOU CAN HELP

FOR PEOPLE WHO NEED MEDICINES

1. Review the RxP Patient Information Card  with people who may need help getting their medications.

2. Help them fill in their clinic and pharmacy phone numbers.If needed, help them call their clinic or pharmacy.

3. Help with medication pickup when transportation or other barriers make it difficult to get their medicines.

FOR COMMUNITY SUPPORT

1. Share RxP Patient Information Cards (English or Spanish) with community organizations, clinics, schools, faith communities, and other groups that support people who may need help accessing medications.

2. Share the Rx Pathway website: medheq.org/RxPathway. Individuals and organizations can download single Patient Information Cards. For bulk printing, please contact us directly. 

3. Connect people with Mutual Aid Support through Indigenous Roots when additional support may be helpful. Call 651-395-7145or email info@indigenous-roots.org.
