Rx Pathway Implementation Guide
Dear Colleague and Friend, Thank you for all you do to support our patients’ health and medication needs. We truly value your partnership! If you have not already, please take less than two minutes to take our RxPathway survey. This survey helps us build a secure registry of resources featuring different stakeholders including individual, family, clinic, hospital, pharmacy, or trusted community aid groups for delievering prescriptions to the most vulnerable patients. Our top priority is patient safety. Because fear of immigration enforcement is keeping many people from seeking care, we do not share any protected health information on social media or public platforms. You can find implementation guides for Rx Pathway below. The one-page implementation guides include instructions on how to pilot the RxP within your own sphere of influence across different stakeholders. Please review them and consider how they may be useful in your setting. The Patient, Clinic, and Pharmacy implementation guides are especially relevant for healthcare teams. The Organizations Providing mutual aid and Neighbors helping Neighbors are helpful for your networks such as family, schools, faith groups, and community organizations. Working together across our networks will help ensure medications reach neighbors who need them during this challenging time.
