This survey helps us build a secure registry of resources featuring different stakeholders including individual, family, clinic, hospital, pharmacy, or trusted community aid groups for delievering prescriptions to the most vulnerable patients. Our top priority is patient safety. Because fear of immigration enforcement is keeping many people from seeking care, we do not share any protected health information on social media or public platforms. 

You can find implementation guides for Rx Pathway below. The one-page implementation guides include instructions on how to pilot the RxP within your own sphere of influence across different stakeholders.
